Each of the following statements are in reference to the pet listed above. Please initial below each of the following statements to confirm your understanding and compliance.

1. I will take my pet to a licensed veterinarian for a checkup within 14 days of this adoption. If the veterinarian certifies that this pet is not adoptable due to severe illness or life threatening congenital defect, I may receive an immediate refund of paid adoption fee once the pet has been returned to Match K-9 Rescue.(Required)

2. I will always provide proper health care for my pet and obtain immediate veterinary care should my pet ever becomes sick or injured.(Required)

3. I will consistently provide my pet with monthly heart worm preventative.(Required)

4. I will provide my pet with proper food, fresh water, adequate shelter, and kind, humane treatment at all times.(Required)

5. I will keep a collar on my dog, with tag bearing my current phone number and I will obey all ordinances pertaining to pets.(Required)

6. I will keep my pet under restraint at all times to prevent him/her from running at large, being a public nuisance or becoming lost or stolen.(Required)

7. I will always contact Match K-9 Rescue & Rehab immediately if my pet becomes lost or stolen, and will make every effort to recovery my pet.(Required)

8. I will not have any type of cosmetic surgery (IE. ear cropping, tail docking, piercing, tattoos, etc.) performed on my pet.(Required)

9. I affirm that this pet is not being adopted as a gift for anyone else, and I assume full responsibility for this pet.(Required)

10. I will never sell, trade or loan my pet out.(Required)

11. I will never allow my pet to be used for experimental purposes.(Required)

12. If, at any time I can no longer care for my pet, I will notify Match K-9 Rescue.(Required)

13. I agree that Match K-9 Rescue may take possession of my pet at any time if: 1) in their opinion my pet is not being properly cared for, 2) if I have misrepresented myself on the adoption application, or 3) if I violate any part of this contract. In the event that I fail or refuse to return my adopted pet on demand, I hereby agree to pay all reasonable attorneys' fees and any court costs Match K-9 Rescue may incur in the reclaiming process of the above pet.(Required)

14. Liquidated Damages: I agree to pay Match K-9 Rescue and Rehab the sum of $600.00 as liquidated damages in the event the terms of this contract are breached. This liquidated damage value is agreed to for the purpose of establishing the value of the animal and does not bar Match K-9 Rescue and Rehab from seeking the return of the animal by a judicial process or other legal means.(Required)

15. I do hereby declare that I am aware (a) that animals are different from human beings in their responses to human actions; (b) that the actions of animals are often unpredictable; (c) that animals should be closely supervised when they are with children; (d) that an animal's behavior may change after he/she gets accustomed to a home environment; and (e) that Match K-9 Rescue& Rehab makes no guarantees as to the temperament, health or mental disposition of any animal put up for adoption.(Required)

16. I hereby accept possession of, title to, and responsibility for the animal listed above and hereby release and discharge Match K-9 Rescue & Rehab, Inc. forever from liability of any injury or damages to any person or property caused in the future by said animal, and from any cause of action, claims, suits or demands whatsoever that may arise as a result of such injury or damages.(Required)

“By signing below I verify the following: 1) I am 18 years of age or older, 2) I am of sound mind and understand all the above requirements, and 3) By signing this contract I must comply with all of the above requirements for the duration of my pets’ life.
