The Foster to Adopt process allows a family or individual the ability to take an animal into their home prior to adoption or in the rare instance that spay/neuter has not yet been completed.

Each of the following statements are in reference to the pet listed above. Please initial below each of the following statements to confirm your understanding and compliance.

During the Temporary Foster Process:

1. I understand that this animal will remain the property of Match K-9 Rescue until the adoption has been finalized.(Required)

2. Adoption must be finalized no more than __ days after Fostering begins.(Required)

3. I understand that many dogs need time to adjust to living in a new environment. This time varies per individual.(Required)

4. I understand that during this adjustment period this pet many startle easily, show signs of being unsure, and/or want to retreat or bolt.(Required)

5. I understand that providing this pet with a crate or personal space during this adjustment period is necessary.(Required)

6. I agree to never allow this pet to be off leash during the foster period.(Required)

7. I agree to use only positive reinforcement during the foster process.(Required)

8. I understand that during this adjustment period this pet may have accidents inside the house.(Required)

9. I agree to feed and potty this pet per the instructions of Match K-9 Rescue while fostering.(Required)

10. I will provide proper health care for this pet and contact Match K-9 Rescue immediate should this pet become sick or injured.(Required)

11. I agree that Match K-9 Rescue may take possession of this pet at any time if: 1) in their opinion my pet is not being properly cared for, 2) if I have misrepresented myself on the adoption application, or 3) if I violate any part of this contract.(Required)

12. I will return this pet to Match K-9 Rescue if I feel it is not a good fit for my family.(Required)

13. I do hereby declare that I am aware (a) that animals are different from human beings in their responses to human actions; (b) that the actions of animals are often unpredictable; (c) that animals should be closely supervised when they are with children; (d) that an animal's behavior may change after he/she gets accustomed to a home environment; and (e) that Match K-9 Rescue& Rehab makes no guarantees as to the temperament, health or mental disposition of any animal put up for adoption.(Required)

14. I hereby accept possession of and responsibility for the animal listed above and hereby release and discharge Match K-9 Rescue & Rehab, Inc. forever from liability of any injury or damages to any person or property caused in the future by said animal, and from any cause of action, claims, suits or demands whatsoever that may arise as a result of such injury or damages.(Required)

*By signing below I verify the following: 1) I am 18 years of age or older, 2) I am of sound mind and understand all the above requirements, and 3) By signing this contract I must comply with all of the above requirements for the duration of the 7 day foster.
