Each of the following statements are in reference to the pet listed above

1. I will take my pet to a licensed veterinarian for a checkup within 14 days of this adoption date. If the veterinarian certifies that this pet is not adoptable due to severe illness or life threatening congenital defect, I may receive an immediate refund of paid adoption fee once the pet has a been returned to Puppy Protection.

2. I will always provide proper health care for my pet and obtain immediate veterinary care should my pet ever become sick or injured.

3. I will consistently provide my pet with monthly Heartworm prevention.

4. I will provide my pet with proper food, fresh water, adequate shelter, and kind humane treatment at all times.

5. I will obey all ordinances pertaining to pets.

6. I will keep my pet under restraint at all times to prevent him/her from running at large, being a public nuisance or becoming lost or stolen.

7. I will always contact Puppy Protection immediately if my pet becomes lost or stolen, and will make every effort to recover my pet.

8. I will not have any type of cosmetic surgery (i.e. ear cropping, tail docking) performed on my pet.

9. I affirm that this pet is not being adopted as a gift for anyone else, and I assume full responsibility for this pet.

10. I will never sell, trade or loan my pet out.

11. I will never allow my pet to be used for experimental purposes.

12. If at any time I can no longer care for my pet, I will notify Puppy Protection.

13. I agree that Puppy Protection may take possession of my pet at any time if: 1) in their opinion my pet is not being properly cared for, 2) if I have misrepresented myself on the adoption application , or 3) if I violate any part of this contract.

14. In the event that I fail or refuse to return my adopted pet on demand, I herby agree to pay all attorneys fees and any other court costs Puppy Protection may incur in the reclaiming process of the above pet.

15. Liquidated Damages: I agree to pay Puppy Protection the sum of $1,000 as liquidation damages in the event the terms of this contract are breeched. The liquidation damage value is agreed to for the purpose of establishing the value of the pet and does not bear Puppy Protection from seeking the return of the pet by a judicial process or other legal means.

16. I do affirm that I am aware (a) that animals are different from human beings in their response to humane actions; (b) that the actions of animals are often unpredictable; (c) that animals should be closely supervised when they are with children; and (e) that Puppy Protection makes no guarantees as to the temperament, health, or mental disposition of any animal put up for adoption.

17. I hereby accept possession of, title to and responsibility for the animal listed above and hereby release Puppy Protection forever from liability of any injury or damages to any person or property caused in the future by said animal, and from any cause of action, claims, suits, or demands whatsoever that may arise as a result of such injury or damages.

By signing below I verify the following: 1) I am 18 years of age or older, 2) I am of sound mind and understand all of the above requirements, and 3) By signing this contract I must comply with all of the above requirements for the duration of my pet’s life.
